Our Personal Care Services provide sensitive, respectful assistance with the private tasks of daily life, funded seamlessly through your NDIS plan, Home Care Package (HCP), or private care arrangements. Delivered by fully qualified care professionals, this service includes discrete support with bathing, dressing, grooming, oral health, and toileting or continence management. We also provide safe mobility transfers and morning or evening tuck-in routines. By matching you with consistent, compassionate workers who strictly honor your personal preferences, we ensure you can start and end your day feeling clean, safe, and completely comfortable in your own home.

DVA Community Nursing Support

Cultural and Language-Sensitive Disability Support

We provide culturally responsive services that respect the diverse backgrounds of clients: language support, culturally appropriate care practices, and community engagement that honours individual preferences.

 

Support for complex care needs

Specialised services for clients with high‐level or complex needs: multiple clinical interventions, high dependency nursing, coordination of multiple service providers, technology or equipment support at home.

 

Clinical care planning and coordination

We develop and coordinate individualized care plans, liaise with GPs, allied health and other care providers, schedule and review visits, and ensure all parts of the client’s care are cohesive and aligned with their goals.

 

Palliative and end-of-life support

Compassionate nursing and support services for clients and families during palliative care or end-of-life phases: symptom management, emotional and spiritual support, and assistance with comfort measures at home.

 

Health monitoring and vital signs assessment

Routine nursing monitoring of vital signs (blood pressure, heart rate, respiratory rate, oxygen saturation, temperature) and interpreting trends to support early intervention and maintain wellness in a home setting.

 

Post-hospital discharge support

Focused care for clients returning home from hospital: medication monitoring wound care, vital signs monitoring, mobility support, and liaising with primary care or specialists to ensure a smooth transition.
